Our process is simple, supportive, and designed to get meals to your door quickly. While anyone can complete the initial inquiry form, official referrals my be completed by a verified HCBS Case Manager. The refferal forms will be sent directly to case managers.
Step 1, Submit Inquiry: Inquiries can be submitted by anyone. We will contact the person on the initial submission to get the case managers' info if it is not included. Your case manager submits your Service Authorization (SA) and basic intake details.
