Who qualifies for Medicaid Home-Delivered Meals?

Individuals enrolled in Colorado Medicaid waiver programs who have an approved Service Authorization (SA) for Home-Delivered Meals (HDM) qualify for meals at no cost. Your case manager or care coordinator can determine eligibility and begin the referral process.

How do I start receiving meals?
  • You can ask your case manager to send your referral anytime.

    • Your case manager submits your Service Authorization.

    • We review your dietary needs and delivery instructions.

    • You begin receiving weekly deliveries.

  • Can I choose my meals each week?

To keep things smooth and safe, our kitchen prepares a set weekly menu created with dietitian guidance.
    However, we can accommodate:

    • Low-sodium

    • Diabetic-friendly

    • Renal-conscious

    • Gluten-aware

    • Soft or chopped textures

    • Certain allergies (when possible) * Please note we cannot accommodate any nut allergies at this time.

Do meals arrive hot or cold?

Meals are delivered chilled, not hot, to ensure safety and freshness. Each meal includes clear reheating instructions.
What if I’m not home during delivery?

    We can deliver to:

    • A cooler you leave outside

    • A designated drop-off location

    • A caregiver or family member

Just let your case manager know your delivery preferences.

How many meals do clients receive each week?

The number of meals you receive depends on the total approved on your Service Authorization (SA).
Your case manager determines this based on your care plan.
Are your meals nut-free or allergen-free?

Important: Our meals are prepared in a shared commercial kitchen that handles peanuts, tree nuts, wheat, dairy, eggs, soy, and sesame. Although we take care with ingredient separation, we cannot guarantee the absence of cross-contact.

Please consult your case manager or healthcare provider if you have severe allergies.
    Do you offer texture-modified meals?

    Yes. We offer:

    • Soft texture meals

    • Chopped meals

Your case manager can request texture modifications during the intake process.

    Who prepares the meals?

Meals are prepared in our local Boulder kitchen by trained chefs following strict safety standards and dietitian guidance.
